Our health care team will work closely with patients and caregivers in an effort to avoid complications and prevent further health deterioration. They will receive continuous education in the latest developments in health care so that Excellence Home Health can maintain its commitment of providing skilled home health care to its clients in a supportive environment.

What is Home Health Care?
A team of health professionals under the request and ongoing directions of the patient's personal physician, is available to bring health care services to homebound patients.
Who may request Home Health Care?
Excellence Home Health Provider Inc.
Provides works on referral basis. Referrals typically originate from a physician, any local hospital, doctor's office, board and care, skilled nursing facility, patient, family or any interested person or agency. Our nurse or representative receives the referral information, then the patient is assessed for the various services the physician requests.

Who pays for these services?
Approved services are covered 100% by Medicare or Medi-Cal. Private insurance coverage varies with individual policies. We do a thorough research of insurance coverage and process all claims.
